The WHO recommends 60 minutes a day of moderate-to-vigorous intensity, mostly aerobic, physical activity, across the week and that sedentary time should be limited. Cycling embodies all of this. &nbsp;Let's explore how trikes, particularly the Rifton Adaptive Tricycle and Schuchmann Momo Trike, play a crucial role in making cycling a reality for children facing unique challenges.The Path to Inclusion:The International Classification of Functioning, Disability, and Health (ICF) Framework, combined with the F-Word ICF Framework by Rosenbaum and Gorter, provides a comprehensive lens to incorporate physical activity seamlessly into the lives of individuals with disabilities. Fitness for all, prioritizing function, fostering friendships, involving family, and infusing fun become the guiding principles.The Joy of Riding:Adaptive trikes are becoming increasingly popular with families and therapists. The National Disability Insurance Scheme (NDIS) recognises the holistic life needs, including recreation and leisure.Therapeutic Benefits of Cycling:Recent clinical research underscores the therapeutic benefits of cycling, with gains in muscle strength, joint range of movement, and bone health. This has also led to demonstrated improvements in gait parameters such as walking distance, speed, and weight shift in stance as the cyclical nature of the movement of pedaling a bike/trike mimics the lower limb movement of walking and is even believed to use some of the same neural pathways of gait. These gains along with improvements in balance, gross motor function, overall improved physical activity levels and cardiovascular fitness make cycling a great activity choice.Links to secondary benefits such as those in the psychosocial realm of confidence, participation, and engagement, add another layer of positivity to the cycling experience.Rifton Adaptive TricycleThe Rifton Adaptive Tricycle was one of the original adaptive trikes developed for children with disabilities who couldn’t ride a traditional bike or trike. Since the release of this trike, there have been many models of adaptive trikes developed to meet various needs of riders. Rifton has also been listening to the feedback from therapists and consumers alike on ways to improve the trike. Some amazing new features have come out of this, and the Rifton trike now meets an even greater population base. All while still retaining the recumbent biking positioning that is the easiest to use for those with very limited capabilities.<figure class="wp-block-image size-large"><img src="https://4030603.app.netsuite.com/core/media/media.nl?id=22813942&amp;c=4030603&amp;h=wOrRimFS_j8lCGCShL1q753-f1cxuPL0W_J9XBBUvsRSuUwF" alt=""/></figure>The new Rifton Adaptive Trike features include:<ul><li>Additional sizes: now available in small, medium and large.</li><li>Increased weight capacity on the large from 90kg to 140kg, and on the medium from 72kg to 113kg. This is a result of design changes on the frame itself.</li><li>Small and large trunk supports with chest harness available on small and medium trikes.</li><li>Small and large swing-away laterals making postural support and transfers easier.</li><li>Variable pedal resistance and a park brake.</li><li>Low friction track for ease of height adjustment of the seat.</li><li>Carbon fibre drive belt will not rust or stretch so reduced maintenance.</li><li>Rear wheels and front end can be removed easily for transport.</li><li>Abductor with adduction straps for controlling leg movement.</li><li>Pelvic harness for firmly positioning the pelvis and providing a stable base for postural control.</li></ul>Add to the already extensive list of accessories available to support even the most physically challenged participant and Rifton is a great choice of trike that will last for years of happy exploring in the world of cycling.Schuchmann Momo TrikeFor upright riding on what looks like a more traditional bike or trike, it would be hard to go past the versatility, robustness and function of the Schuchmann range of trikes and bikes.The Momo Trike has a compact design with a low, wide frame for easy independent transfers, and an &nbsp;extensive range of postural supports that make trike riding possible for even those with complex support needs.<figure class="wp-block-image size-large"><img src="https://4030603.app.netsuite.com/core/media/media.nl?id=22813941&amp;c=4030603&amp;h=E1_fMeH_tMw6M4mJ1q141xg4XWqS5FnI8_sJBI2a7uYUxWdy" alt=""/></figure>What sets the Momo trike apart though is the availability of choice of drive type, with standard rigid (allowing for freewheeling), a 7-Speed gear hub version, and even the option of electric support. Add to this a range of sizes from 12” to 26”, and both the smallest and largest riders are able to access the world of supported trike riding.The Follow Me Momo is a clever parent-child tandem coupling system to connect the Momo Tricycle to any adult bicycle. From the initial stages of development, the progression is directed towards achieving an upright stance—beginning with the ability to roll over in the first few months, followed by crawling at approximately 9 months, and culminating in the act of pulling oneself to stand at around 10 months. This developmental trajectory is driven by the overarching aim of attaining the distinct perspective and autonomy afforded by independent standing and mobility.Moreover, assuming an upright position plays a pivotal role in facilitating the optimal functioning of various physiological components, including bones, muscles, internal organs, and the nervous system. This fundamental alignment not only reflects the natural course of human development but also underscores the significance of an upright posture in supporting overall health and functionality.<div class="wp-block-group">If we examine the musculoskeletal system, the benefits of standing are multiple:<div style="height:20px" aria-hidden="true" class="wp-block-spacer"></div><div class="wp-block-group"><ul><li>Promotion of Hip Stability: Standing contributes to the development of the hip joint, fostering better foot and knee alignment. Notably, children with cerebral palsy (CP), though initially possessing normal hip anatomy (Mayson T. 2011), face an increased risk of hip displacement due to limited standing (Hägglund G. 2014). Development of extensor muscle strength, with an opportunity for enhancement of postural control of the head and upper body and reduction of the risk of atrophy.</li><li>Development of Extensor Muscle Strength: The act of standing offers an opportunity to enhance extensor muscle strength, thereby improving postural control of the head and upper body. This engagement also diminishes the risk of muscle atrophy.</li><li>Spasticity Management: Standing has been observed to effectively manage spasticity, leading to a notable reduction in the effects of tone after the activity (Paleg et al 2013).</li><li>Enhancement of Bone Mineral Density: Regular standing contributes to improved bone mineral density, consequently reducing the risk of fractures (Uddenfeldt, 2013) (Kim S, 2017).</li><li>Reduction of Contracture Risk and Improved Range of Motion: The upright position reduces the risk of contractures and improves the range of motion compared to being seated in a flexed position (Paleg et al 2013). &nbsp;</li><li>Promotion of Spinal Alignment: Standing promotes spinal alignment, thereby reducing the risk of orthopedic deformities such as hip dislocation and scoliosis. This, in turn, helps alleviate associated pain.</li><li>Enhanced Musculoskeletal Growth and Development.</li></ul></div></div><div style="height:20px" aria-hidden="true" class="wp-block-spacer"></div>From a participation view, being upright can show improvements in:<div class="wp-block-group"><ul><li>Functional reach and hand function</li><li>The opportunity for improved head control and ease of eye contact</li><li>Normalisation of field of vision and enhanced visual field input</li><li>Age-appropriate position to interact with peers and adults, providing enhanced social development</li><li>Psychological well-being; psychosocial benefits of positioning at peer-level; improved self-concept</li><li>Increased alertness, with standing stimulating the limbic/reticular activating system.</li></ul></div><div style="height:20px" aria-hidden="true" class="wp-block-spacer"></div>Various factors can contribute to delayed standing or the need for additional support during the standing process. These delays or requirements for support may stem from the effects of neurological conditions, motor impairments, or genetic factors.In the case of children presenting with diverse neurological conditions falling under the umbrella term Cerebral Palsy (CP), the utilisation of standing frames is considered a best practice, particularly for those classified at GMFCS levels IV and V.Research findings within this population endorse the use of standing frames, emphasising the importance of abduction of at least 30 degrees (Martinsson C. 2011) (Paleg G. 2013) for stable hip development, especially among those with the most limited mobility within their programme.Standing Frame Options:When we look at the options for supported standing, there are broad categories of standers, with the choice depending on the capabilities of the user and their support needs.<ul><li>Vertical/Upright&nbsp;Standers have the user positioned fully upright in the frame and requires a certain degree of trunk and neck postural control and the ability to use an assisted standing transfer. This is generally where children who only need a little bit of support may start their exploration into being upright.</li></ul><figure class="wp-block-image size-large"><img src="https://4030603.app.netsuite.com/core/media/media.nl?id=21850356&amp;c=4030603&amp;h=k2kJHPUDobqkMgrhM1sOW7lZbC-8Dx0IV-MSlJ96c3LSVJpv" alt=""/></figure><ul><li>Prone&nbsp;Standers have a slightly forward tilted position, offering slightly more support than the upright stander but still needing head control for use. An assisted standing transfer is still generally required too but this depends on the degree of forward tilt used. They are excellent for stimulating extension of the hip and trunk and improving reach capacity with the upper limbs.</li></ul><figure class="wp-block-image size-large"><img src="https://4030603.app.netsuite.com/core/media/media.nl?id=21850357&amp;c=4030603&amp;h=QsLOGFH50nMEhHE1xC7sFvAak7tpQh3AXknAxoz08edpPDS1" alt=""/></figure><ul><li>Supine&nbsp;Standers&nbsp;are typically used for individuals with poor head and trunk control.&nbsp; The posterior trunk support together with the use of rearward tilt facilitates an upright position with graduated changes towards more upright standing.&nbsp;&nbsp;Vertical height adjustment can make transfers into supine standers easier if using more heavily assisted transfers or hoisting and provide versatility&nbsp;and easy adaption for individualised higher postural support needs.</li></ul><figure class="wp-block-image size-large"><img src="https://4030603.app.netsuite.com/core/media/media.nl?id=21850354&amp;c=4030603&amp;h=1U0RTlVR88ctm8pS9rYA0_sphQE8-kJqU2j4EFoZL3M6texo" alt=""/></figure><ul><li>Multi-Positional Standers are a single stander which offer the benefits of ease of change from supine to prone and upright. Many models now also incorporate adjustable hip abduction as well. The human body is inherently structured for an upright posture. Overall, there are likely several goals for wheelchair users and their families as well as the clinical considerations of their therapy team to take into account.Power Wheelchairs can open up a world of possibilities for those who experience difficulty with mobilising and being able to reposition when seated. &nbsp;However, the effectiveness of the powerchair to achieve all the goals (clinicians and clients), is dependent on the choices made at the time of scripting.Essentially it will come down to:<ul><li>Clinical Factors – Identifying the needs of the client and risks associated with not meeting each need. Then, deciding what is non-negotiable (such as pressure relief, postural control, and drive access) and what can be compromised (additional features such as standing function, multiple transport options, or all-terrain access).</li><li>Lifestyle Factors – environment of use and transport considerations. What does their everyday look like and where do they need to go?</li><li>Personal Choice – what is a “need to have” Vs “nice to have”? Do any of these choices compromise any of the non-negotiables?</li></ul>This should all come together to provide a picture of what powered mobility needs to provide for the client. It is important to note that this will be unique to each client despite possibly having similar diagnosis and demographics to others.<figure class="wp-block-image size-large"><img src="https://4030603.app.netsuite.com/core/media/media.nl?id=20734137&amp;c=4030603&amp;h=9KmOii8DBwgQ2CzhDN3djaiTc7KYX3F09yYxIs7IDPsK3IzO" alt="A person in a wheelchair and a person in a wheelchairDescription automatically generated"/></figure>Scripting:The order of the below choices is still up for debate as they all influence each other. These are the broad areas for consideration, which all have an impact on the outcome for your client.The Seating:A good place to start is knowing what is required to meet the postural and pressure needs of the client. This is key to ensure you can make the decision on which type or model of power wheelchair is going to be suitable. For this, you should consider:<ul><li>The level of postural support required. Will the use of an integrated seat (low level of support) with no power seat functions meet this need? Or is a specialised seating system with high support required to meet complex needs?</li><li>Complex seating will need to be mounted to the power wheelchair and will likely be used with power seating functions. How will the chosen seating system mount to the powerchair?</li><li>If there are any other seating options such as head or lateral supports, belts, harnesses, etc, that will impact on mounting.</li><li>How the materials of the cushion interact with the suspension capabilities of the powerchair.</li></ul>Keep in mind that a great seating prescription can be undone by not correctly matching to a suitably scripted powerchair.Power Seating Functions:Knowing which power seat functions are required is also key when prescribing seating on a power wheelchair. The options are tilt, recline, seat elevate, foot elevate, and standing, with combinations selected based on client need (determined at initial assessment). Not all power wheelchair models with powered seat function have all these features. It is vital to know if the power wheelchair selected has the functions required by the client and that they work in a way which is easily accessible. &nbsp;Access:During the initial assessment, a decision would have been made as to how the client is going to control the chair. This will include the consideration if proportional or non-proportional controls will be used and which type of each will be selected. Access options are varied across manufacturers with additional options also available. Knowing that some power bases drive better when alternative controls are used over other chairs will also help set up for success. Being clear at the start as to what you are needing to achieve and if customisation is required will guide your selection.Castor Selection:There is usually little choice of castor on a particular model of powerchair, therefore this should be considered when making a choice of chair model at the start. Differing options of size, width, material, and suspension in the castor arm can make a huge difference to how a chair drives, the feel of the ride, and how well it manoeuvres.The Power Base and Drive wheel position:All power wheelchair bases have their pros and cons, and you will also need to consider the motor capabilities of each model to make the best choice. It comes down to, “What do you need the chair to do?&nbsp; What goals are you trying to achieve?”.&nbsp; Remembering that a compromise is likely to be needed, so understanding what is most important to the client should remain front of mind.Rear Wheel Drive is great for driving in outdoor community spaces and performs well at higher speeds. It is stable with some climbing ability and generally has good ground clearance. It also offers great control with non-proportional inputs. However, it does have the biggest footprint and users generally need to have good spatial awareness to manoeuvre the chair as the drive wheels are behind them. This could be overcome safely with some training and experience but is a good reminder that using a rear-wheel-drive power chair takes some skills and experience to learn to drive it. A rear wheel drive also has poorer traction on steep downhill slopes.Front Wheel Drive has great climbing ability and small front castors which means the user can get close to desks. &nbsp;Smaller castors also means that tighter foot placement can be achieved. It does however have poorer control with non-proportional inputs, or when travelling at higher speeds. Similar to rear wheel drive, users generally need to have good spatial awareness to manoeuvre the chair with the drive wheel now out in front of them.Mid Wheel Drive is more intuitive to drive as you are turning from where you are sitting. This also means it has the smallest turning radius, and therefore the smallest footprint of the group. It does however require the use of front and rear stabilisers and castors which can impact on transfers and front access. Awareness of this is required as they can have a tendency to get “hung” on dips such as curbs if approached on the wrong angle. Overall, a mid-wheel drive power base can suit most clients and their needs with the greatest potential of overall use. It is also the most prescribed power base set up.Four Wheel Drive / All Terrain. These powerchairs are often larger with the compromise being reduced manoeuvrability to enable off-road capabilities. &nbsp;These are generally not a good choice for indoor mobility unless you have a large home, wide hallways, and hard flooring; however they are amazing for opening up the opportunities for access in the great outdoors. &nbsp;<figure class="wp-block-image size-large"><img src="https://4030603.app.netsuite.com/core/media/media.nl?id=20734139&amp;c=4030603&amp;h=snBTzqGXvd-ZMX1xArWRwwG9l1EXVcCY59RSOWq1BeOk5d3y" alt="A collage of different wheelchairsDescription automatically generated"/></figure>Seating and power base considerations:The use of power seat functions will also influence the seat base to power base positioning and the chair’s functioning. For example, recline and tilt will both shift the balance of the client’s weight on the power base. Therefore, knowing their drive position is incredibly important and should be accounted for when putting the whole picture together.Suspension:Along with the considerations of base is the question of suspension, how this is achieved on the base, and how it interacts with the seating. Every chair is different so check the ride quality over the surfaces the client is going to be accessing. For example, a Roho cushion might be indicated for pressure relief but can be unstable when paired with an all-terrain option such as the X8 and used on rough walking tracks. Tyre Selection:The tyres on a powerchair form a part of the suspension package of the powerchair base. These may also determine, to some extent, the terrain the user is going to be able to access. Selection is therefore very important, and knowing which powerchairs offer which options is handy to know. A smaller, narrower solid tyre will give you amazing manoeuvrability on flat surfaces inside, but will struggle over gravel or grass outdoors. &nbsp;Likewise, a wider, pneumatic, low-pressure tyre is great for traction over rough surfaces and gives a softer ride but can make turns inside a struggle if the turn space is tight. There’s also the debate over maintenance of a pneumatic tyre versus a solid or hybrid tyre. There are obvious advantages to a solid tyre but remember to consider ride feel. &nbsp;There is little advantage in scripting a solid tyre if this then affects the feel of the ride to the point the client can’t use the chair.Set Up:Now that you’ve made all the choices on the script for a powerchair and seating it should all just come together, right? In fact, this is where the skills of an experienced therapist and/or an AT specialist really come to the fore.For example, placement of the seating base on the power base in relation to the drive wheels has a big impact on the balance of the chair. Too much weight over the castors will cause it to drive and turn heavily. There is also a concern with the chair becoming “tippy” towards the front when encountering curbs and slopes. Powerchair scripting – the functional effects of choices

There are many secondary benefits of this set up which additionally preserve and aim to maintain independence in other areas of daily life and wellbeing, too.A systematic review in 2016, explored factors influencing participation of wheelchair users in their social and community environments. The scope to change outcomes by addressing some factors or accommodating for others was also identified. When looking at the “modifiable” factors, there is a wide range where the use of a power add-on unit can improve participation and access. Some examples include:<ul><li>being able to provide effective propulsion in the presence of reduced hand grip strength.</li><li>managing energy expenditure by augmenting or replacing the need for manual propulsion.</li><li>managing fear of risk /risk of shoulder injury with sustained and repetitive manual propulsion.</li><li>Reducing dependence on others by providing opportunity for independent mobility.</li></ul>These primary benefits then promote greater opportunity for participation and access and many secondary benefits can be gained. Particularly contributing to being engaged with the community, motivation to access community environments and preservation of shoulder health for independence in daily living tasks and overall, helping to achieve positive life outcomes.What about exercise and fitness?Occasionally, a question is raised on using a power add-on unit and removing the opportunity for long distance self-propelling, and with it, the perceived potential benefits on cardiovascular and respiratory function. There is little evidence outlining the impact on cardiovascular or respiratory function when comparing manual self-propelling against the use of a power add-on unit. There is no statistically significant data that shows using a power add-on unit negatively impacts a user in this way when compared to self-propelling, however further research continues to be recommended. What is available though, is extensive research on shoulder health for manual wheelchair users who self-propel, and the risks associated with sustained, repetitive manual propulsion. There is a high prevalence (30-70%) of manual wheelchair users with a spinal cord injury sustaining a repetitive strain injury in their shoulder (Gil-Agudi et al, 2014). &nbsp;It is also important to note that propulsion biomechanics does not differ significantly between athletic users (those with significantly greater levels and intensity or physical activity per week) and non-athletic users in their daily manual wheelchair (S.J, Briley, et al, 2020).If weighing up the perceived potential benefits of long-distance self-propelling against use of a power add-on unit, consider the known risk to shoulder health and the complications that can arise from an injury impacting function and quality of life.<figure class="wp-block-image size-large"><img src="https://4030603.app.netsuite.com/core/media/media.nl?id=20234280&amp;c=4030603&amp;h=pmPmHxiDseFhsoIXg34vmGg5vqP89j86C--zrW_lLUovYl77" alt=""/></figure>Don’t forget to consider the frame and system set upIt is also important to note that using a power add-on unit is only one out of five ways to address the need to preserve shoulder health and function for manual wheelchair users. Attention and consideration of the whole system should be reviewed and using a power add-on unit on a sub-optimal system should be avoided. Some power add-on units significantly increase the demands on the frame. Check the strength, durability and vibration dampening properties against the chosen unit and environment/s of use. Careful consideration of wheelchair frame components can also promote efficiency of the whole system or cause barriers and sub-optimal use.Where to start to narrow down the optionsWith multiple options currently available for consideration, how do you start to identify a suitable unit? Power adds on units can be broken down into 2 categories:<ol type="1"><li>those that offer partial assistance (augmenting the user’s manual propulsion) such as rear mount (Alber Smoov, SmartDrive, Klaxon Twist), hand-rim activated (Alber E-Motion) and front mount with no handlebar (Klaxon Twist).</li><li>those that provide full assistance (removing the need for manual propulsion), such as joystick conversion (Alber E-Fix, Todo, i-Xpress) and front mount with handlebar (Klaxon Klick or Twist).</li></ol>It is a good idea to consider this first, after evaluations and discussions have occurred between the clinician and the user.  This collaborative approach can help ensure the needs and capabilities of the user along with the parameters of the unit are suited.  It will also look into how this set up will achieve the desired goals and outcomes in the intended environment/s of use. For example, does the front castor need to be lifted off the ground to overcome barriers in the community.Following this, the type of set up can be narrowed down. Most units will offer one type of set up, either rear mount, hand rim activated, joystick control, or front mount with handlebar and one which offers a combination depending on the goals and intended environments of use.<figure class="wp-block-image size-large"><img src="https://4030603.app.netsuite.com/core/media/media.nl?id=20234279&amp;c=4030603&amp;h=M-66o11hrr_WADXjJvkcxFdDNxowVGvEo-EcgCJoK-WCaMYw" alt=""/></figure>A note on transitioning from one environment to another with a power add-on unitConsidering if a manual wheelchair user is changing environments throughout the day and if power is needed in each environment, could highlight the need for a power add-on unit that offers multiple set ups. For example, consider the need to access outdoor community areas and then office spaces, places of learning, shopping centres, or even air travel and use on a trip. There is also evidence to support an emerging trend for user transitioning from one environment to another and finding themselves limited when the power add-on unit is not suitable for use in the new environment. Funding bodies will often only approve one type of power add-on unit. In looking to optimise a user’s engagement and participation, having an option that can cross over from one environment to another can help achieve this.
